Working alongside colleagues in Westminster City Council, we have formed a Bi-Borough partnership which supports the sharing of facilities and expertise – this allows us to provide joined-up health and social care services that are constantly evolving and improving.
With both boroughs ranked as having some of the best adult social care services in the country, we intend to maintain these high standards, supporting people to stay close to their communities and homes, and outside of acute and institutional care where it is appropriate. We have strong values and a focus on improving outcomes to vulnerable adults and their carer’s and we work closely with our community and health colleagues to improve people’s lives.
We are looking for Advanced Practitioner Social Workers to join our Adults, Complex Team, working 4 days a week. Supporting the Team Manager, this role will be responsible for providing professional support and direction to social care staff in the Integrated Health and Social Care Team. There will also be a reduced case load. You will play a vital role in promoting quality, best practice and person-centred care in the service provision for Adults with physical disabilities and Older People in the Borough. There will also be a cross-over with mental health and other areas of social work.

As an Advanced Practitioner, you will need:
• A recognised Social Worker qualification
• Relevant professional registration (Social Work England)
• Experience working with vulnerable adults and/or children within a health and social care setting
• Experience of providing staff or student supervision.
