The Role:
Working closely with acute hospital trust and community partners, you will develop an innovative and creative service that delivers choice and independence to local residents. A key challenge will be leading and managing a team of Social Workers and a Discharge Support Worker, with your co manager, as well as managing and controlling budgets for the provision of high-quality health and social care. Able to assess risk and make sound decisions, you will also ensure effective safeguarding at every step.

About You:
A self-starter and a problem solver you’ll bring new ideas and energy to this well-established and highly professional team, working with them to shape future services. Exceptional communication and collaboration skills are essential to build rapport and coach others towards success.
With a background and appropriate qualifications in health or social care, you’ll have demonstrable experience of leading and managing multi-agency teams to ensure effective service delivery. Knowledge of the legislative and statutory guidance underpinning social work and social care is essential. A highly experienced people manager, you’ll have the tools and skills to measure and improve performance.
